Definitions:

  • Summary: A brief description or title of your issue
  • Details: Provide as many specifics about your issue as possible.
  • Incident Address: What is the address where the issue occurs? This may or may not be your home address. It isn't mandatory to provide an address, but if your issue is specific to a location then an address or intersection is critical to our ability to help solve your issue.
  • Submit: Create your case.
  • Submit and Search Solutions: This button creates your case PLUS it will use your summary description to search our Solutions Library to see if an answer to your issue already exists.
