All permanent employees are eligible to enroll in a supplemental vision plan through Superior Vision. This plan may be used alone or in conjunction with any of the below medical plan benefits. Active enrollees will receive a vision I.D.card from Superior Vision to show at the time of the service.

Copayments:
- $10 Exam
- $25 Eyeglass Lenses and/or Frames
Plans Services/Frequency:
- Exam: 12 months
- Lenses: 12 months
- Frames: 24 months
- Contact Lenses: 12 months*
*Contact Lenses are in lieu of eyeglass lenses and frames benefit.
If you are covered under one of the City and County of Denver's medical plans, your vision exam is available through your medical provider. Enrollment is automatic with your medical election.
