Update My Personal Information

Throughout the year, the Controller's Office, Payroll Division may find it necessary to mail documents to your home or mailing address. Some examples include:

  • Your paycheck
  • Your W-2 Form
  • Other official documents
To ensure that our records are up to date, you may view and update your:
  • Home and Mailing Address
  • Personal Phone Numbers
  • Emergency Contacts
  • Personal Email Addresses
  • Marital Status
  • Other Personal Information
Electronic Viewing and Updating My Personal Information
 
*please note, you must be able to log on to DOT to perform these actions
 
  1. Log on to DOT
  2. Click on the My Page tab
  3. From the Self Service Center choose My Profile
  4. Choose View/Edit Personal Information
  5. Find the category that you would like to update and click correct
  6. Edit your information
  7. Click Save

You will receive an automatic confirmation that your data has been saved

 
If you do not have access to DOT, you may submit a Change My Personal Information form to the Payroll Division.

Name change
 
To have your name changed, please supply the Controller's Office, Payroll Division with a copy of your new social security card with your new name. Please also notify the Payroll Division if this name change comes with a new marital status. We will pass this information along to CS records.
 
 

Name Change

To have your name changed on your paycheck, please supply the Office of Human Resources with a copy of your new social security card with your new name. 

To have your name changed in the Microsoft Outlook universal address book, please contact Technology Services. To have your name changed on your phone, please make a separate request to Technology Services.
