Free Mammogram Screening Day

Getting your mammogram this year can be easier, more convenient, and most importantly, a life saver. Don’t think you have time? The Denver Wellness team wants to help, by bring your mammogram appointment to you. Attend one of the free Mammogram Screening Day at Denver Human Services with The University of Colorado Hospital mobile mammogram bus. It only takes 20 minutes to keep you on the path to being well. The event will provide free, onsite mammograms for employees who are 40 and over*, and due for their annual exam. You must bring a photo ID, insurance card, and be a member of United Healthcare or Denver Health to receive the screenings.

**Please note: Kaiser Permanente appointments have all been filled. Kaiser Permanente members can also schedule a free mammogram at a medical office by calling 303-338-3456 weekdays, 8 a.m. to 5 p.m. (TTY for deaf, hard of hearing, or speech impaired: 303-338-4428). Saturday and same-day appointments available to make scheduling even easier.

The Healthy Steps Incentive Program is up and running, so get an early start on those annual exams!!


Kaiser Permanente Members go to this website. Sign in with your normal KP username and password to see what screenings you need to do.   As you complete your screenings you will see big green check marks, letting you know you are done! Don't forget to take the Total Health Assessment! Get one preventative dental cleaning between November 1, 2014 and October 31st 2015!


 United Healthcare Members Print the Health Action Form and take it to your annual physical and for your doctor to fill out as you complete your screenings. Go to this website to take your Health Risk Assessment and make sure that UCH received the form from your doctor. Get one preventative dental cleaning between November 1, 2014 and October 31st 2015!


 Denver Health Members Take the Physician Reporting and Biometrics Form to your annual physical and for your doctor to fill out as you complete your screenings. Go to this website to take your Health Risk Assessment. Get one preventative dental cleaning between November 1, 2014 and October 31st 2015!

Forms will be available to download February 26th, 2015.

Once you have completed the requirements, Denver Wellness will send you a congratulatory email, letting you know that you are finished!

Dental Requirement:

Kaiser Permanente Members: Check off the dental requirement when you have completed your visit. Denver Wellness will randomly select employees who say they have completed all of the requirements including the dental visit for audit.
United Healthcare Members:
1) If you have the City's Delta Dental Insurance, you will not need to do anything to report your dental visit. Denver Wellness receives a list of employees who completed their visit on a regular basis and we will notify you when you have completed all of the requirements.

2) If you DO NOT have the City's Delta Dental Insurance, you will need to send the dental reporting form to Denver Wellness with a signature from your dentist's office. 
Denver Health Medical Plan Members:
Take the Physician Reporting and Biometrics Form with you to your dentist appointment and have your dentist sign and date the section titled "To Be Completed By Dentist." Denver Wellness will randomly select employees who say they have completed all of the requirements including the dental visit for audit.

Complete these steps by October 31st, 2015!
If you do not have insurance through the City, you are not eligible for the incentive this year. You must be employed by the City in December, when the incentives are disbursed, in order to receive the incentive.

Weekly Stress Relief Classes at Human Services! Denver Wellness presents Boost Your Zen—a blend of practical stress reduction, mediation techniques and realistic methods to improve your perspective in difficult situations. Employees learn to quickly change unhealthy habits with simple modifications that have a significant impact in their overall wellbeing. Through ongoing reinforcement and practices that can be done in a few minutes every day, participants achieve sustained change towards wellness success. Every Tuesday at noon at the Castro Building! Starting at the Webb Building on April 16th, at noon! Go to City U to register!

TIAA CREFF offers webinars for every financial situation. Whether you need to pay off your student loans, get started on retirement savings, or plan a household budget, there's a course for you.

Our partner, the Denver Community Credit Union, offers online resources and onsite classes to help you better undrestand your finances. Check City University for classes at the Webb Building in 2015.

Financial Empowerment Centers provide one- on- one financial couselling to get you through any financial issue-- from buying your first car, to putting your kids through college, contact the FEC to make an appointment at 720-944-2498 and check out their multiple locations at http://www.mpoweredcolorado.org.

Random Acts of Wellness is our way of celebrating wellness achievements great and small! Just snap a picture of yourself doing something for your well- being and post it to our Facebook page (or email it to us and we'll post it for you) to be entered to win our monthly R.A.W. drawing! Your selfie can be doing just about anything for wellness, whether its running in a 5k, attending a wellness educational event or participating in a wellness campaign, as long as it has to do with your well being, it's in!
