Follow three easy steps by October 31st, 2014! Start the year right by striving to be your best!

1. Make an appointment with your primary care physician for your annual check up.

2. Complete the online health assessment through your city sponsored health care provider (Denver Health, Kaiser Permanente or UnitedHealthcare).

3. Schedule and report the required tests for your age and gender.

Kaiser members will only need to make an appointment with their primary care provider if certain screenings are out of date, otherwise Kaiser members can schedule directly with the medical office providing the screening (for example, if a member needs a colonoscopy, he would make an appointment with the Gastroenterology Department). Visit your online medical record for more information about your screenings.

United and Denver Health members who have visited their doctors since October 1, 2013, will still need to have their reports signed by a physician.

 If you do not have insurance through the City, you are not eligible for the $240 incentive this year. You must be employed by the City in December, when the incentives are disbursed, in order to receive the incentive.
