A lien release is required for all active liens. The lien release must be on the lienholder's letterhead (letterhead is not required if lienholder is an individual). Photo and fax copies are accepted and must include vehicle year, make, VIN, titled owner's name(s), agent's signature, date of lien release and must be signed under penalty of perjury in the second degree as defined in C.R.S. 18-8-503.
The duplicate title will be issued omitting all reference to the lien pursuant to C.R.S. 42-6-126.
DR 2539A, Duplicate Title Request Form, In order to apply for a duplicate title, the original owner can apply at any county office within the State of Colorado. The named owner must present a secure and verifiable identification for duplicate title applications with a puchase date on or after July 1, 2006.
If someone other than a named owner wishes to apply for a duplicate title a named owner will provide a non-secure power of attorney
DR2175, Non-secure power of attorney to his/her agent to include the grantor's (named owner's) secure and verifiable identification information for vehicles with a purchase date on and
after July 1, 2006.
If someone other than a named owner wishes to apply for a duplicate title for vehicles with purchase dates
prior to July 1, 2006 a named owner will provide a
DR2175, Non-secure power of attorney to his/her agent to include the grantor's (named owner's) identification information.
Note: The agent is required to present secure and verifiable identification regardless of the purchase date of the vehicle.
