Public Health Inspection
Licensing
Both emergency medical companies and emergency medical vehicles are required to be licensed by the City and County of Denver. Companies that are located outside of the county boundaries but operate vehicles inside the county are required to be licensed. Applications for licensing are processed through the Department of Environmental Health prior to Excise & Licensing’s approval.
Applications must be complete and fees collected before the multi county coordinator is called to approve the vehicle inspections.
Licensing Requirements
Whether a company is renewing a current license or applying for a new license the following items must be included in the annual application packet.
- Signed and Notarized Application
- Proof of Insurance
- Professional liability $1 Million each person
- Professional liability $2 Million each accident
- Property Damage $1 Million each accident
- Bodily Injury $1 Million each person
- Bodily Injury $1 Million each accident
- Vehicle Condition Certificates
- List of emergency medical vehicles including unit numbers and license plate numbers.
- Drug list signed and dated by the physician advisor
- List of personnel including training & certification records
- Schedule of rates and hours
- Applicable fees. Checks should be made payable to the "Manager of Revenue".
New Business Licensing Requirements
Emergency medical companies that have not been licensed before or companies that have allowed their license to expire are required to complete the following additional items:
- A one time license application fee of $150.00 (in addition to the licensing fee of $105.00)
- Proof of zoning if the company is in Denver County
- Finger prints. The responsible party will be finger printed by Excise & Licensing.
Licensing Fees
Checks must be included in the application packet and made payable to the "Manager of Revenue".
- The yearly license fee is $105.00
- Emergency Vehicles are $50.00 per vehicle
- Non Emergency Vehicles (any surface vehicle hired to transport sick or disabled persons providing nonemergency medical services) are $30.00 per vehicle
- For new applicants there is an application fee of $150.00
