Contact and Feedback Information

Denver’s Community Health Improvement Plan (CHIP) has been developed with extensive community involvement, and constitutes a community-wide plan for improving health in the city. This community involvement will continue throughout the implementation period, with organizations and individuals helping to realize the goals and objectives outlined in the CHIP Action Plan. Organizations in Denver can assist in this process by aligning their programs, activities and resources with the Access To Care and Healthy Eating and Active Living goals and objectives. With such an alignment, these organizations can also benefit from the ongoing CHIP evaluation that will be carried out in the coming years by the Be Healthy Denver team to measure progress in their own programs.
