In the November 1, 2011 Coordinated Election, candidates and the proponents and opponents of a ballot issue are permitted to appoint poll watchers.

To appoint poll watchers, use the Excel spreadsheet (preferred) to list your appointees. If you cannot use a spreadsheet, you may hand fill out the PDF form.

• Report of Major Contributors  -- This form must be completed by any candidate committee that receives a contribution or contribution In-Kind of $500 or more within six days preceding the election. It is due on the next business day.

 Municipal Candidates:

• Municipal Candidate Acceptance Forms -- Fill in the Mayor, Council, Auditor or Clerk and Recorder acceptance form, as appropriate. This form must accompany completed petitions when turned in for validation.
• Municipal Candidate Affidavit -- Due within ten days of becoming a candidate. 
• Political Committee Treasurer Affidavit -- Due within ten days after formation of a political committee. 
• Statement of Campaign Committee Organization -- Due within ten days after formation of a political committee.
• Candidate Financial Disclosure Form -- Due within 15 days of becoming a candidate. This includes incumbants who filed a City Officer Financial Disclosure Form.
• Candidate / Campaign Contact Sheet -- Denver Elections contacts candidates using information provided on this form and also shares campaign links with the public on our website. 

School Board Candidates:

• School Board Candidate Statement of Intent -- Must be turned in to the Elections Division in order to obtain petition blanks.
• Candidate / Campaign Contact Sheet -- Denver Elections contacts candidates using information provided on this form and also shares campaign links with the public on our website.
• Affidavit of School Board Candidate -- This affidavit of qualifications and acceptance of nomination must accompany your completed petition.
