Marriage License Forms

• Designated Beneficiary Agreement form - This form must be notarized.
• Large print Designated Beneficiary Agreement form - This form is 3 pages, adding $5.50 to the recording fee. See our Fees page. This form must be notarized.
• Revocation of Designated Beneficiary Agreement form - This form must be notarized.
