All Classification and Compensation forms are fillable Adobe PDF forms. As such, when you download a file you must first save the file on your computer. Please using the following format for your file name:
Agency/DepartmentName_EmployeeID_EmployeeName_Process (i.e. IndividualPositionAudit or PayEquityAdjustment)
All Classification and Compensation forms must be completed digitally, signed digitally and communicated for signature and submission via email. This will create a digital file path to ensure efficient and timely processing of your process request.

Pay Equity Adjustment Form
A pay equity adjustment is an adjustment to an employee’s pay to maintain competitive pay within a work group/unit or classification. Read more about this form.
Pay Equity Adjustment Request Multiple Employees
When requesting a pay equity adjustment for multiple employees in a work group/unit or classification within a department/agency please complete this form. The addendum form allows for additional consideration of up to four more employees per addendum. Please complete an additional addendum form for every four additional employees being considered for a pay equity adjustment.

Pay Increase Upon Voluntary Demotion Form
An employee may voluntarily demote to a different position, explained in Career Service Rule 9-33 and is initiated by the appointing authority with the Office of Human Resources executive director's approval.
Signature Addendum Form
Each Classification and Compensation form requires signature approval based on Career Service Rule. If the department/agency requires additional levels of approval other than those required by Career Service Rule, complete the signature addendum form. The signature addendum form allows departments/agencies to designate up to ten additional levels of required signatures. The signature addendum form may be attached to any Classification and Compensation form to complete the department/agency's internal processes.
