Sample Restaurant Inspection Form
Use this option to view the items contained on the restaurant inspection form. This form will show you all of the possible violations that are currently reviewed during a regular inspection. You may click on any of the violations to read about the public health significance for that violation. We hope that you find this section both informative and educational.

Search (Inspections or Enforcement Actions)
This option allows you to search for a particular restaurant by establishment name or street address and review the inspections or enforcement actions for that restaurant since June 1, 2000.

Public Health Inspection Division Web site
Use this link to go to the Denver Public Health Inspection Division Web site, which contains information about food safety, filing complaints, food manufacturing, food peddlers, illness caused by food, outdoor special events, smoking ordinance, food labeling requirements and other valuable information.
