Basic Life and Accidental Death and Dismemberment Insurance
 
The City provides a Basic Life Insurance policy on your behalf. Your enrollment is automatic and the premium is paid for entirely by the City. The following chart highlights the benefits of these policies depending upon your employment status:

Three-Quarter & Half Time
(hired prior to Jan 1, 2002)   


2x annual salary, maximum $75,000
 
Three-Quarter Time
(hired after Jan 1, 2002)
 

1.5x annual salary, maximum $75,000
 
Half Time
(hired after Jan 1, 2002)
 

1x annual salary, maximum $50,000
 
Sheriff - Uniformed    
 
2x annual salary, maximum $400,000

Enrollment in the Basic Life Insurance benefit is automatic; however, you should keep your beneficiary designation current. Completed Basic Life beneficiary designation forms can be returned to the OHR Benefits Office.

Additional Life Plans
 
You may also elect Additional Life Insurance coverage for yourself and Spouse life coverage for an eligible spouse in units of $5,000 to a maximum benefit of $300,000.
If you are a new employee, you may elect up to $100,000 for yourself and $30,000 for your spouse if you enroll within 30 days of hire without being subject to medical underwriting approval.
 
If you are beyond your first 30 days of employment or want to elect more than $100,000 for yourself and/or $30,000 for your spouse, you and/or your spouse must complete a Medical History statement. Standard Insurance Co. may also require a physical examination in order to be granted the additional coverage. 

You may also elect at any time:
  • Accidental Death and Dismemberment (AD&D) Insurance for yourself and your family in increments of $10,000 up to $500,000
  • $5,000 or $10,000 Dependent  Life Insurance policies for your eligible children (under the age of 26)

No medical history statements are required for enrollment in these policies.
