Do You Know Your Health Numbers? 

The Third year of the Denver’s Wellness Incentive Program is underway. One of the requirements of the DenverWellness Incentive Program is completing an online health assessment through your health care provider.

 

Health Assessment

A health assessment provides employees with concrete information about their current health risks and overall health status. Based on the results of your health assessment, recommendations will be given in order to improve your health and quality of life. 

 

To get started with your online health assessment, we recommend that you know your four key health numbers.

  1. Blood pressure
  2. Blood sugar
  3. Cholesterol
  4. Body Mass Index (BMI)

 

These numbers are key indicators of your risk for major chronic illnesses. Employees are encouraged to use their health care providers or community health fairs to obtain current health numbers. Once you receive your health numbers, the information is used to complete the online health assessment through your City-sponsored health care provider. Inserting your 4 health numbers into the health assessment will assure a more accurate and individualized health report. 

 

Obtaining Your Health Numbers

  • Kaiser – Kaiser members may call their doctor’s office to request an order for blood work at the lab, there will be no charge (one time per year). Employees may then schedule a wellness health check and discuss the results of your blood work with your doctor.   
  • United Healthcare – United Healthcare members may call their doctor’s office to request an order for blood work, there will be no charge (one time per year). Employees may then schedule a wellness health check and discuss the results of your blood work with your doctor. 
  • Denver Health – Denver Health medical plan members may call their doctor to request an order for blood work, there will be no charge (one time per year). Employees may then schedule a wellness health check and discuss the results of your blood work with your doctor. 
  • Community Health Fairs – The 9Health Fair is the most well known health fair in Denver and will be held at various locations from April 21 – 29, 2012. They offer blood work for a fee and several other health screenings.  Community health fairs are also held at local churches, schools and health clinics.

 

Employees who complete the Denver Wellness Incentive Program will receive a $144 credit in their 2013 pay checks. Employees have until August 31, 2012 to complete their health assessment, one approved Physical Activity Program and the Nutrition Challenge, a Healthy Plate.  For instructions on how to complete your online health assessment or for more information about the Denver Wellness Incentive Program, visit www.denvergov.org/denverwellness or call (720) 913-5687/
