Accident/Incident Investigation Standard - 65.9.1

The purpose of this Standard is to ensure that all accidents and incidents that result in a workplace injury or illness are thoroughly investigated to identify causal factors, and to ensure that corrective actions are taken to prevent a reoccurrence.  The procedure in this Standard provides guidance to supervisors for completing the investigation and the City and County’s Supervisor Report of Accident or Incident.
