In 2015 Medical Enrollment is Automatic

Due to health care reform, all full time employees hired as of January 1, 2015 will be automatically enrolled into the lowest cost medical plan (Kaiser DHMO) with coverage effective the first of the month following the hire date. See section 4 of the 2015 Employee Benefits Guide for details on waiving, changing or adding dependents to this medical coverage.

The following is a summary of the medical carriers and plans we will be offering in 2016:

  • Medical insurance carriers will remain the same in 2016: Denver Health, Kaiser and UnitedHealthcare
    The City is introducing a new type of medical plan called a high-deductible health plan (HDHP) in the place of the HMO offered in 2016. Along with the same medical providers in 2016 as in 2015 come the same covered services, the same Rx coverage and the same network of doctors and facilities. The only thing different is how you pay for your health services. 

  • 2015 DHMO plans continue in 2016. Click here to view the summary.
    The Denver Health and Kaiser Deductible HMO (DHMO) plans, and United’s Navigate plan offered in 2015 will remain the same in 2016. However, the premiums for these DHMO plans will increase in 2016.

  • Introduction of high-deductible health plans (HDHP). Click here to view the summary.
    To replace the HMO plans, the City will offer HDHP plans through each of the current medical providers: Denver Health, Kaiser and UnitedHealthcare. HDHPs have the same covered services and networks as the 2015 HMO plans, but replace copays with deductibles and coinsurance. These HDHPs also qualify to be paired with Health Savings Accounts (HSA).

  • Medical Plan Cost Estimator: Use this medical plan cost estimator to help determine what medical plan works best for you and your family in 2016.

  • Health Savings Account (HSA) An HSA is a pre-tax medical savings account used to pay for qualified expenses. An HSA can only be used when enrolled in a high-deductible health plan (HDHP). In 2016 the City is pre-funding every account with $600 for those enrolled in an HDHP with single coverage and $1200 for those enrolled in an HDHP with a dependent.

    Unlike the flexible spending accounts, you own your account and there are no “use it or lose it” restrictions. HSAs allow you to save and “roll over” funds from year to year. Even if
    you change health plans or jobs, the money in the account is yours to keep, including the City’s 2016 contribution to your HSA.

ENROLLMENT FOR 2016 IS ACTIVE!
Active enrollment means that you will be required to make an election to ensure you have your preferred plan (or opt out) in place for 2016. If an election is not made, you will be automatically defaulted into the Kaiser HDHP plan with employee only coverage.

How do I ENROLL?
1. Log into DOT and click on “2016 Benefit Enrollment” between 10/5/15 and 10/30/15
2. No access to DOT, complete your 2016 Benefits Enrollment form mailed to your home address and return to Benefits via:

- Mail – OHR Benefits, 201 W. Colfax Ave., Dept. 412, Denver, CO 80202
3. REMEMBER—enrollment is required!

New employees must submit the benefits 2015 Enrollment Form* along with supporting documentation (if enrolling dependents) to the OHR Benefits Office within 30 days of their hire date. This form can be used to enroll in medical**, dental, vision, flex cash and prepaid legal. Any benefit elections made will become effective on the first of the month following your hire date.

Please note: all new employees will be automatically enrolled into the Kaiser DHMO medical plan with employee only coverage unless this coverage is waived, another medical plan is elected or dependents are added within the first 30 days of employment.

 
The dependents listed below are eligible for coverage under our medical, dental, and vision plans:
  • Spouse
  • Spousal Equivalent (same-sex domestic partner)
  • Child (eligible until end of  month of age 26)
  • Step-child (eligible until end of month age 26)

Family Status Changes
If your family status changes, you have the option to update your benefit enrollments (medical*, dental, vision, flex cash). Family status changes include:

  • Birth or adoption of a child
  • Death of a dependent
  • Marriage or divorce
  • Change in hours worked
  • Gain or loss of other coverage

To update your benefits, complete the benefits 2015 enrollment form** within 30 days of the status change, and submit it to the OHR Benefits Office. Please also include proof of the status change and the required supporting documentation if you are enrolling new dependents.

The six medical plans offered to eligible City employees are Denver Health Medical Plan's (DHMP) HMO and Deductible HMO, Kaiser Permanente HMO and Deductible HMO (DHMO), United Healthcare HMO and Navigate Plan. All six medical plans are effective the first of the month following your date of hire. Additionally, Dental, Vision, Short-term Disability and Flex Cash are effective the first of the month following your date of hire. Your life insurance and long-term disability become effective the first day of work.


 

Denver Health Medical Plan (DHMP)
As a member of DHMP, you receive your care through the doctors at 10 community health centers throughout Denver.  All locations have free, easily accessible parking for DHMP members. Several convenient pharmacy options are available, including online prescription refills and Pharmacy Delivery by Mail, offering a 90-day prescription supply for one co pay. To learn more about DHMP visit www.denverhealth.org or call member services at (720) 956-2100.

Kaiser 
Kaiser Permanente is the largest nonprofit health plan in the United States. They are an integrated health delivery system, which means they provide and coordinate the entire scope of care for members, including: preventive care, well-baby and prenatal care, immunizations, emergency care, screening diagnostics, hospital and medical services, pharmacy services. United Healthcare Choice Copay Plan
The UnitedHealthCare Choice Copay plan is similar to a traditional HMO which coordinates your care through a primary care physician and allows you to pay a low flat fee (copayment) for most covered services. To find out more about UnitedHealthCare Choice Copay Plan, visit www.welcometouhc.com/cityandcountyofdenver or call 1-800-842-5520. This is the City's only medical plan that offers full coverage outside of Colorado. The plan's summary is linked below

UnitedHealthCare Navigate Plan
Members enrolled in this plan must select a Primary Care Physican (PCP) and the network of physicians is limited to only Colorado.  In exchange for lower premiums, members may be required to pay a deductible and coinsurance for certain services. See this plan's summary below.

Delta Dental of Colorado is the dental provider for the City and County of Denver. Delta offers three plans through the City and County of Denver the Exclusive Panel option and two Delta Preferred Provider Options.

Delta Preferred Provider Option II (PPO) High
  • You have the option of obtaining services from a PPO (in-network) dentist or a non-PPO (non participating or Delta Premier network dentist) with a $2000 annual benefit.

Exclusive Panel Option (EPO)

  • You make a copayment  for covered services. Plan highlights include: $10 exams, fixed costs for all other procedures and no annual out-of-pocket maximum. , employees are eligible to enroll in a supplemental vision plan through VSP Vision. This plan may be used alone or in conjunction with any of the medical plan benefits listed below. 

*Please note, active enrollees will not receive a vision ID card. VSP's vision providers will verify coverage using the enrollee's social security number at the time of service.

If you are covered under one of the City and County of Denver's medical plans, your vision exam is available through your medical provider. Enrollment is automatic with your medical election.

To help protect your income in the event of an injury or illness, the City offers you short-term and long-term disability benefits. Standard Insurance Company is the City and County of Denver's short-term and long-term disability plan provider.  

Short-term Disability (STD)

If you were hired after January 1, 2010, or you converted to the City's Paid Time Off (PTO) plan, the City pays the full cost of the premiums for your short-term disability insurance. You are automatically enrolled into this benefit and it is effective upon your hire date. This benefit includes the following:

City-paid STD: Weekly benefit: 70% of salary with $1500 maximum per week 
Waiting period: 14 days
Monthly cost: $0 (City-paid)


If you were hired by the City and County of Denver prior to January 2010, or you work in the District Attorney's office or for the Sheriff's department, you are eligible to voluntarily enroll into one of the five STD plans listed below. 

Available voluntary STD Plans:

Plan 1: Maximum weekly benefit: $350 
Waiting period: 7 days
Monthly cost: $23.63

Plan 2: Weekly benefit: 70% of salary with $1500 maximum per week 
Waiting period: 7 days

For these plans the premiums are paid entirely by the employee. You will have the opportunity to make a change to your STD enrollment every year during open enrollment. However, if you elect outside of your new hire period a late enrollment penalty (LEP) applies. A LEP means that if you file a claim for anything other than an accidental injury during the first 12 months after your coverage becomes effective, STD benefits will become payable after you have been continuously disabled for 60 days and remain disabled.

Long-term Disability (LTD)
Long-term disability (LTD) insurance is designed to pay a monthly benefit to you in the event you are unable to work for after 180 days due to a covered illness or injury. 
  • Your long-term disability becomes effective the first day of work.
  • The monthly long-term disability benefit is 60% of the first $10,000 of your monthly pre-disability earnings earnings.  The City pays 100% of the premium and you are automatically enrolled upon hire.

*PLEASE NOTE: 24HourFlex has sent out new 24HourFlex debit cards. These new red 24HourFlex cards are being issued to replace your existing blue cards. The old blue cards were deactivated on November 3, 2014. You will need to activate your new red card beginning on November 10, 2014.

Basic Life and Accidental Death and Dismemberment Insurance
 
The City provides a Basic Life Insurance policy on your behalf. Your enrollment is automatic and the premium is paid for entirely by the City. The following chart highlights the benefits of these policies depending upon your employment status:

Enrollment in the Basic Life Insurance benefit is automatic; however, you should keep your beneficiary designation current. Completed Basic Life beneficiary designation forms can be returned to the OHR Benefits Office.

Additional Life Plans
 
You may also elect Additional Life Insurance coverage for yourself and Spouse life coverage for an eligible spouse in units of $5,000 to a maximum benefit of $300,000.
If you are a new employee, you may elect up to $100,000 for yourself and $30,000 for your spouse if you enroll within 30 days of hire without being subject to medical underwriting approval.
 
If you are beyond your first 30 days of employment or want to elect more than $100,000 for yourself and/or $30,000 for your spouse, you and/or your spouse must complete a Medical History statement. Standard Insurance Co. may also require a physical examination in order to be granted the additional coverage. 

You may also elect at any time:
  • Accidental Death and Dismemberment (AD&D) Insurance for yourself and your family in increments of $10,000 up to $500,000
  • $5,000 or $10,000 Dependent  Life Insurance policies for your eligible children

No medical history statements are required for enrollment in these policies.

Due to the importance of utilizing alternative transportation, the City participates in and subsidizes the cost of the Eco Pass and ValuPass programs with the Regional Transportation District (RTD).
 
Both programs allow employees to use RTD's bus and light rail services as alternative transportation to and from work.

  • Pay for commuting expenses with pretax dollars
  • No parking issues
  • Peace of Mind provided by Guaranteed Ride Home Program sponsored by Denver Regional Council of Governments  (Eco Pass only)

Eco Pass
The Eco Pass is an RTD-issued photo ID card called a smart card. The monthly cost is $39 (pretax) and is deducted from the 2nd paycheck of each month.     

Enrollment Provisions
Eligible employees may enroll at anytime:
  • Eco Pass is available to full-time City employees.  Part-time employees may contact the Benefits Team for transit pass options.
  • City employees at Denver International Airport (DIA) are not eligible for the Eco Pass program, but can enroll in the 2015 DIA Valupass program.

To enroll, employees must see a benefits representative in the OHR Benefits office, located in the Webb Municipal Building, 4th Floor, to receive an RTD smart card authorization form. ( Note: Employees located at Denver Human Services and Denver Public Library may obtain Eco Pass enrollment details from the Human Resources offices on-site.)      

You may cancel your Eco pass participation at any time. To cancel, please complete the RTD termination form and return as directed on the form.

Your ability to retire is very important to the City. To make sure your retirement years are successful, the City offers a full range of retirement benefits.
 
In order for you to have enough resources in retirement, the City provides three different retirement options.
  • Denver Employee's Retirement Plan (DERP)
  • Social Security
  • Deferred compensation (or 457 Plan)

DERP
All permanent employees are automatically enrolled in the Denver Employee's Retirement Plan (DERP), also known as the Plan. The Plan is a pension plan which provides a lifetime monthly retirement benefit for all vested City Employees.
 
In general, you are eligible to receive these benefits after 5 years of continuous service and upon reaching the age of 65. The benefit varies based on your salary and length of service.
 
As of January 1, 2015, the City and County of Denver and DHHA contribute 11.5% of the employee’s total gross salary to the trust fund, and the employee contributes 8% of his or her total gross salary. Employee contributions are made on a pre-tax basis through payroll deductions.

The lifetime monthly benefit you will receive from the Plan is intended to provide a portion of your retirement income. Please visit www.derp.org to learn more about the Plan


 

Social Security
Both you and the City contribute to Social Security. The benefit for social security varies.  Visit www.socialsecurity.gov for more information. 


 
Deferred Compensation (457 Plan)
  

The deferred compensation plan, or 457 plan, is the City’s voluntary retirement savings plan. A 457 plan is similar to a 401(k) plan in the private sector. However, unlike some 401(k) retirement plans, the City does not match employee 457 plan contributions. 

Deferred compensation is a way for you to set aside money via payroll deduction on a pre-tax basis to save for retirement and defer, or delay, paying any taxes on the money you invest, as well as, any earnings from those investments. 

Some other benefits of investing in a 457 plan are:

  • the pre-tax deductions lower your taxable income
  • you are taxed on this money when you retire and are more likely to be in a lower tax bracket
  • the convenience of automatic payroll deductions
  • Unlike a 401(k) or 403(b), there is no 10% penalty for withdrawal before the age of 59 1/2 (although the withdrawal is subject to ordinary income taxation). This means you have immediate access to the funds if you leave the City for any reason.
  • You may enroll, increase, decrease your contributions at any time.
  • You may also contribute to the 457 plan through after-tax contributions into a Roth account.

Contributions to the 457 plan can only be made through payroll deductions. You decide how much to contribute and you can contribute as little as $10 per check or as much as 100% of your total pre-tax pay. Keep in mind, the IRS sets annual contribution limits for 457 plans. The current 2015 annual contribution limits are as follows:

  • Under 50: $18,000
  • 50 catch-up: $24,000
  • Pre-retirement Catch-up: $36,000

The City and County of Denver provides several types of paid leave to help employees achieve a work/life balance:
  • Paid Time Off (PTO) or Sick and vacation
  • Bereavement leave
  • Holiday and other leave

Paid Time Off (PTO)
PTO offers flexibility to employees in accessing and managing time off because it provides one leave bank that can be used for both vacation and sick leave purposes.

Eligibility

Eligible employees hired or re-hired after December 31, 2009 and employees hired prior to January 1, 2010 who elected to convert from the sick and vacation leave plan to the PTO plan.

Sick and Vacation
The sick and vacation leave plan provides two separate banks, one for sick leave and one for vacation leave.

Eligibility

Eligible employees hired prior to January 1, 2010 who remain continuously employed by the City and who did not voluntarily elect to receive PTO benefits.

Bereavement Leave

Employees who receive PTO benefits shall be entitled to use up to forty (40) hours of paid bereavement leave because of the death of a member of the employee’s immediate family. This forty (40) hours of bereavement leave shall not count against the employee’s PTO bank.

Employees in the sick and vacation leave plan may use up to forty-eight (48) hours of their accrued sick leave hours due to the death of a member of the employee's immediate family. 



Holiday Leave

The City provides eleven (11) paid holidays per year to eligible employees.
  • New Year's Day (January 1);
  • Martin Luther King Day (third Monday in January);
  • Washington's Birthday (third Monday in February);
  • Cesar Chavez Day (last Monday in March);
  • Memorial Day (last Monday in May);
  • Independence Day (July 4);
  • Labor Day (first Monday in September);
  • Veterans' Day (November 11);
  • Thanksgiving Day (fourth Thursday in November);
  • Christmas Day (December 25);
  • Personal holiday (one (1) personal holiday on a date agreed upon by the employee and the City to be used within the calendar year). 

Other Paid Leave
Eligible employees may also be eligible for paid court leave, election leave, administrative leave and military leave.

With GuidanceResources, you have an attorney “on call” whenever you have questions about legal matters. Speak with on-staff licensed attorneys about legal concerns such as divorce, custody, adoption, real estate, debt and bankruptcy, landlord/tenant issues, civil and criminal actions and more. If you require representation, you can be referred to a qualified attorney for a free 30-minute consultation and a 25 percent reduction in customary legal fees.

Financial Information
Everyone has financial questions. With your GuidanceResources benefit, you can get answers to your questions about budgeting, debt management, tax issues and other money concerns from on-staff CPAs, Certified Financial Planners® and other financial experts, simply by calling your toll-free number.

GuidanceResources® Online
Go online to access timely, expert information on thousands of topics, including relationships, work, school, children, wellness, legal, financial and free time. You can search for qualified child and elder care, attorneys and financial planners as well as ask questions, take self-assessments and more.


GuidanceResources is available to you 24 hours a day, 7 days a week.

There are two ways to access your GuidanceResources EAP benefits:

1. Call 877.327.3854. You’ll speak to a counseling professional who will listen to your concerns and can guide you to the appropriate services you require.

Benefits Forms

Employees wishing to add an opposite-sex spouse (without a marriage certificate) must complete the Common-Law Affidavit. Employees wishing to add a same-sex spouse must complete the Spousal Equivalent Affidavit.
 

Please Note: If you were previously married and are adding a common-law spouse, you must provide a divorce decree for the prior marriage.

Short-Term Disability
Standard Insurance Company is the carrier for short-term disability. The City has 2 short-term disability plans. One is a voluntary benefit for those employees hired prior to January 1, 2010. This benefit is paid for by the employee through payroll deductions.  The other is an automatic benefit for those employees hired after January 1, 2010. This benefit is paid for by the City.

Long-Term Disability
Standard Insurance is the carrier for the City and County of Denver's Long Term Disability plan. All benefit eligible employees are enrolled and premiums are provided to employees as part of their benefit package.

Basic 
The City provides a basic life insurance benefit to all benefit-eligible employees. The policy is 2 times the employee's annual salary to a maximum of $100,000 (reduced if a 1/2 or 3/4-time employee). Employees need only complete the following beneficiary designation form to ensure the benefit is appropriately distributed.

Additional
Standard Insurance Company is the carrier for Supplemental/Additional Life and Accidental Death and Dismemberment coverage. Employees may elect to purchase up to $300,000 of additional life insurance for themselves and their spouses at group rates. Employees may also elect to purchase up to $500,000 of Accidental Death and Dismemberment coverage for themselves or their family.
 
The privacy rules under the Health Insurance Portability and Accountability Act (HIPAA) require the City andCounty of Denver (the “City”) to provide participants with Privacy Notices for the following Health Plans: the City’s Flex Cash Plan (administered by Denver Reserve Corporation) and the City’s Self-Insured Dental Plan (administered by Delta Dental Plan of Colorado). Each respective Privacy Notice explains participants’ rights under that Plan and the Plan’s legal duties with respect to protected health information (PHI) and how the Plan may use and disclose PHI.
