Benefits

The five medical plans offered to eligible City employees are Denver Health Medical plan (DHMP), Kaiser Permanente HMO, Kaiser Permanente Deductible HMO (DHMO), United Healthcare HMO and United Healthcare Navigate Plan. Medical, Dental, Vision, Short-term Disability and Flex Cash are effective the first of the month following your date of hire. Your life insurance and long-term disability become effective the first day of work.


Denver Health Medical Plan (DHMP)
As a member of DHMP, you receive your care through the doctors at 10 community health centers throughout Denver.  All locations have free, easily accessible parking for DHMP members. Several convenient pharmacy options are available, including online prescription refills and Pharmacy Delivery by Mail, offering a 90-day prescription supply for one co pay. To learn more about DHMP visit www.denverhealth.org or call member services at (720) 956-2100.

Kaiser 
Kaiser Permanente is the largest nonprofit health plan in the United States. They are an integrated health delivery system, which means they provide and coordinate the entire scope of care for members, including: preventive care, well-baby and prenatal care, immunizations, emergency care, screening diagnostics, hospital and medical services, pharmacy services.         UnitedHealthCare Choice Plan (HMO/EPO)
        The UnitedHealthCare HMO/EPO plan is similar to a traditional HMO which coordinates your care through a primary care physician and allows you to pay a low flat fee (copayment) for most covered services. To learn more about UnitedHealthCare Choice Plan, visit www.myuhc.com or call 1 (800) 842-5520.


        UnitedHealthCare Navigate Plan 

        Members will have have acess to a smaller network than those in the traditional HMO/EPO Plan. In exchange for lower premiums, member will be required to pay a deductible and co-insurance for services.         Delta Dental of Colorado is the dental provider for the City and County of Denver. Delta offers three plans through the City and County of Denver the Exclusive Panel option and two Delta Preferred Provider Options.

        Exclusive Panel Option (EPO)
        • You pay a copay for covered services. Plan highlights include: $10 exams, fixed costs for all other procedures and no annual out-of-pocket maximum.
        • You must obtain services from a Delta Dental PPO Panel Dentists. Participating EPO dentists can be found by clicking on 'Dentist Search' atwww.deltadentalco.com, click on the 'Delta Dental PPO' under production selection or you may call Delta at 1 (800) 610-0201.

         Delta Preferred Provider Option II (PPO) High
        • You have the option of obtaining services from a PPO (in-network) dentist or a non-PPO (non participating or Delta Premier network dentist) with a $2000 annual benefit.
        • PPO dentists can be found by clicking on "Delta Dental PPO" under product selection at www.deltadentalco.com.  Non-PPO dentists can be found by clicking on "Delta Dental Premier" or you may call Delta at 1 (800) 610-0201.
        • If you elect to use a non-PPO (Premier) dentist may incur higher out-of-pocket costs.

         Delta Preferred Provider Option III (PPO) Low
        • You have the option of obtaining services from a PPO (in-network) dentist or a non-PPO (non participating or Delta Premier network dentist) with a $1250 annual benefit.

        f you are covered under one of the City and County of Denver's medical plans, your vision exam is available through your medical provider. Enrollment is automatic with your medical election.

        UnitedHealthCare

        • HMO Plan:

        Exam:  One exam every 24 months, $60 copayment

        Eye Wear Coverage:  Discounts on lenses, contact lenses and frames through UnitedHealth Allies Discount program 

        •  Navigate Plan:

        Exam:  One exam every 24 months, $25 copayment

        Eye Wear Coverage:  Discounts on lenses, contact lenses and frames through UnitedHealth Allies Discount program 

        Kaiser Permanente

        • HMO Plan:

        Exam:  One exam every 24 months, $30 copayment.  *Additional charge for  Contact Lens examination and fitting.

        Eye Wear Coverage:  $100  credit toward lenses, frames or contact lenses every 24 months.

        • DHMO Plan:

        Exam:  One exam every 24 months, $30 copayment.  *Additional charge for  Contact Lens examination and fitting.

        Eye Wear Coverage:  No benefit

        Anthem Vision Plan
        Employees  may enroll in a supplemental vision through Anthem Vision.  This plan may be used alone or in conjunction with any of the above plans. Plan and provider information is available at www.anthem.com .  You may also call Anthem at -1-866-723-0515. 

        Exam: One exam every 12 months, $10 copayment

        Eye Wear Coverage

        • Lenses: Covered in full every 12 months, $25 copayment
        • Contact Lenses:  $120 Allowance every 12 months
        • Frames: $100 Allowance every 24 months
        Standard Insurance is the City and County of Denver's short-term and long-term disability carrier.  

        Short-term Disability

        Short-term disability (STD) insurance is a voluntary plan designed to pay a weekly benefit to you in the event you cannot work because of a covered illness or injury. You may only elect STD during your new hire period.  STD benefits may continue during a disability for up to 180 days. 

        If you elect outside of your new hire period a late enrollment penalty (LEP) applies. A LEP means that if you file a claim for anything other than an accidental injury during the first 12 months after your coverage becomes effective, STD benefits will become payable after you have been continuously disabled for 60 days and remain disabled.

        Available STD Plans:
        All plans pay the lesser of 70% of your predisability earnings or the plan's maximum weekly benefit.  

        Plan 1: Maximum weekly benefit: $350 
        Waiting period: 7 days
        Monthly cost: $18.90 

        Plan 2: Maximum weekly benefit: $1500 
        Waiting period: 7 days
        Monthly cost: $.0087 x Gross Monthly Earnings 

        Plan 3: Maximum weekly benefit: $1500 
        Waiting period: 14 days
        Monthly cost: $.0070 x Gross Monthly Earnings 

        Plan 4: Maximum weekly benefit: $1500 
        Waiting period: 30 days
        Monthly cost: $.0053 x Gross Monthly Earnings

        Plan 5: Maximum weekly benefit: $1500 
        Waiting period: 60 days
        Monthly cost: $.0035/Gross Monthly Earnings 

        If you have more questions review the STD Frequently Asked Questions document.  To file a claim, download the STD Claim Form, forward the physician statement to your doctor for completion, complete the employee statement and contact the CSA/Benefit or Controller's office to complete the employer portion.  Short-term disability certificate.

        Long-term Disability
        Long-term disability (LTD) insurance is designed to pay a monthly benefit to you in the event you are unable to work for after 180 days due to a covered illness or injury. 

        The monthly LTD benefit is 60% of the first $10,000 of your monthly pre-disability earnings earnings.  The City pays 100% of the premium and you are automatically enrolled upon hire. 

        Flex Cash is a way for you to set aside money on a pre-tax basis for certain qualified expenses.  There are three plans the City offers:

        • Medical spending - allows you to pay for medical expensed not covered by insurance with pre-tax dollars.
        • Dependent Care spending - allows you to pay for dependent care for your children so you and your spouse can work with pre-tax dollars.
        • Qualified parking - allows you to pay for parking expenses while you are at work with pre-tax dollars. 
        • You may enroll only during open enrollment, within 30 days of your hire date or if you experience a family statuse change. Your annual pledge may not be changed once the election is made.

        There are several benefits to City employees who participate in the program, including:

        Full-time City employees may enroll at anytime with no 12 month contract. The Eco Pass sticker can be picked up at time of enrollment from CSA Benefits.

        • No parking issues
        • Peace of Mind provided by Guaranteed Ride Home Program sponsored by Denver Regional Council of Governments

        Eco Pass Monthly Cost:

        • $39 per month for eligible City and County of Denver employees.
        Enrollment Provisions:
        • Your cost for the program will be taken directly through a monthly pre-tax payroll deduction.
        • The Eco Pass can be picked up at time of enrollment.

        The Wellness Center features a full compliment of fitness equipment, exercise classes and services for all employees.

        The center is open for early morning workouts as well as after work hours and on Saturday. Employees' ID badges are used to allow access to the Wellness Center in the lower level of the Webb Municipal Building.
