There is still time to complete the online Health Assessment as the deadline has been extended to June 30th. To assist in understanding the results of your personalized health report, feedback sessions are scheduled. Employees will have a chance to ask questions, learn about services and programs from the health care providers. Sessions are June 16th in the Webb Building from 12:00 - 1:00pm, 4.G.2, and at Department of Human Services on June 23rd.

 

The questionnaire is designed to provide you with a detailed and personalized health report. The individualized report will show the relationship between your behavior and healthy habits. Also, included in the feedback, will be a plan of action and a listing of online health programs. All information is confidential between you and your provider. Upon completion of the online assessment, print your individualized health summary.  Keep this for your records and to discuss with your doctor.  Aetna members also print your Certificate of Completion. 

All information is confidential between you and your provider. Information you provide will be used and disclosed in accordance with the HIPAA Privacy Rules including using this information in aggregated form only. In other words, the information is combined with data collected from other individuals to produce anonymous, aggregated statistical information.
