We request that you complete this form and provide all necessary paperwork to us as soon as possible so that we may request restitution as part of the defendant’s sentencing.
Please note that restitution is court-ordered reimbursement to a crime victim from a defendant who has been convicted of the crime. In other words, restitution can be ordered by the judge only as a condition of a sentence. The Denver City Attorney’s Office will make recommendations to the court on the amount of restitution request based on documents submitted by you, the crime victim.
