You may use an Emergency Replacement Ballot if you are unable to vote your original mail ballot due to:
- Confinement in a hospital or residence
- An immediate family member’s confinement in a hospital or residence
- Emergency conditions like fire, flood or other catastrophic event
Complete an Emergency Replacement Ballot Form(PDF, 99KB) and send it with your designated representative. Your representative will need to present an acceptable ID along with the signed form to pick up your ballot.
