Case Management

Case management helps people with HIV navigate the world of HIV-related medical and support services. Case management programs provide care coordination and assistance with accessing medical care, social support, financial services, and more. Case managers can help you to navigate the HIV care system in the Denver Metro Area and connect you to resources and services necessary for improving your health and wellbeing.

It Takes a Village

It Takes A Village is a non-profit organization whose mission is to reduce health and social disparities among people of color in the Denver, Colorado metropolitan area. It Takes a Village serves people living with HIV or who may be at risk for acquiring HIV. Financial assistance, case management, linkage to medical care and treatment, mental health counseling, psychosocial groups and support are provided for HIV positive people of all races. Services are provided in English and Spanish.

Tepeyac Community Health Center

Tepeyac Community Health Center is a non-profit community health center that provide culturally competent health care and preventive health services for the medically underserved. Clinica primarily serves that Latinx community and offers services including primary care, dental, behavioral health, and maternal and women’s health.

Servicios de La Raza 

The Servicios de La Raza program, La Gente, is a bilingual program specializing in providing client-centered services to people living with HIV (PLHIV) who reside in Adams, Arapahoe, Denver, Douglas, and Jefferson counties. The program assists its participants to understand their diagnosis, gain and maintain access to medical care, treatment, emotional support and other resources that meet their needs so they may thrive with HIV. La Gente serves a wide range of PLHIV including straight and lesbian, gay, bisexual, transgender and queer/questioning (LGBTQ) men, women, and youth.

Empowerment Program

The mission of the Empowerment Program is to provide education, employment assistance, health, housing referrals, and support services for women who are in disadvantaged positions due to incarceration, poverty, homelessness, HIV/AIDS infection, and/or involvement in the criminal justice system.

Clinic-based

The HIV primary care clinics allow HIV patients to be seen in a primary care office setting alongside other patients seeking care at the location. This provides our patients with anonymity and the comfort of receiving care in a traditional office setting. Our internal medicine physicians and nurse practitioner are specially trained to provide routine medical care services to all populations, and they are highly experienced in HIV treatment, PrEP and transgender health.

The Children’s Hospital Immunodeficiency Program (CHiP) offers research and prevention programs that improve the health and quality of life for children, youth, young adults, and pregnant woman living with HIV. CHiP offers services to families living with, affected by or at-risk for HIV.
