2026 Community Health Improvement Plan

Welcome to Denver’s Community Health Improvement Plan (CHIP)

This plan reflects a collaborative and inclusive planning process that engaged Denver residents, community‑based organizations, healthcare systems, and government partners in creating meaningful health improvement goals. Using the 2025 Denver Community Health Assessment (CHA) as its foundation, this five‑year plan translates community‑identified needs into prioritized goals that mobilize collective action, drive measurable progress, and support long-term health improvement.

Guided by community voices, local data, and the expertise of partners, the CHIP focuses on three priority areas essential to Denver’s overall health and well‑being:

  • Access to Health Care
  • Safe and Affordable Housing
  • Systemic Racism

This page offers a preview of the CHIP’s goals, strategies, objectives, and partners. Click the links below to learn about:

Strategies, objectives, and partner commitments will continue to evolve during a 180-day action‑planning period and throughout implementation, as the CHIP is a dynamic plan intended to be evaluated, revised, and strengthened over time.

DDPHE extends its sincere appreciation to the many Denver residents and partner organizations who contributed their time, insight, and experience to this plan. The collective lived and learned experience of the many CHA-CHIP Coalition members has meaningfully shaped and strengthened this work.
