To improve overall health outcomes for Denver residents, DDPHE identifies and addresses health disparities across the City and County of Denver. Foundational to identifying health disparities and improving overall health outcomes are a Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). The primary objective of the CHA is to comprehensively gauge and assess the health landscape of Denver.
The CHA workgroup at DDPHE is tasked with assessing public health trends to craft strategic interventions aimed at reducing health disparities. This project fulfills the overall objective of the CHA and CHIP to provide digestible public health data through visuals to Denver residents.
The project has the following specific deliverables due by October 1, 2024:
- Develop the following:
- Comprehensive visuals for various social determinants of health topics (e.g., health, socioeconomic status, environmental factors, etc.) with descriptive statistics (e.g., prevalence, count, or incidence) at the census tract level within Denver, Colorado, and U.S. rates, with a margin of error for Denver;
