Who Is This Form For?

This form is intended solely for healthcare and medical service providers, including hospitals, long-term care facilities, clinics, home health agencies, family medicine practices, and nursing homes that are serving Denver in response to COVID-19 and have a specific need for medical-related equipment and/or supplies.

PPE and other medical supplies are prioritized for those providing medical services and healthcare response to the COVID-19 response.

Before requesting supplies from the City & County of Denver, you should attempt to procure the supplies through your normal channels. The city is prioritizing all inquiries for supplies, based on the greatest need, which is evolving rapidly. The City cannot provide assurance that your request can be met. If the City is able to assist, please note that you may receive less than the total amount of your request. Other organizations may be able to assist you in procuring PPE, including organizations such as MasksNOWProject C.U.R.E., and numerous distilleries that are producing sanitizer.
