Who Is This Form For?

This form is intended solely for providers serving vulnerable populations and/or performing essential services in Denver in response to COVID-19 and have a specific need request inquiry for:

  • personal protective equipment (PPE)
  • food or food equipment/service,
  • supplies,
  • and/or other necessary support

Important Note:
The City and County of Denver cannot guarantee that any request will be filled, and the City may not be able to fulfill an entire request. Before requesting help with supplies from the City & County of Denver, you should continue efforts to procure needed items through your normal supply channels.

PPE and other medical supplies are prioritized for those providing health care and medical services to the COVID-19 response.

If you are not a healthcare or medical service provider, please understand that the City and County of Denver may not be able to fulfill your request. 

Who Is This Form NOT For?

  • Individual or family needs 
    If you are an individual or family and have a need or would like to request support services, please visit our Relief Support & Resources page for assistance or call 3-1-1.
  • Requests for small quantities
    If you need small quantities of any of the items above (less than pallet-sized, i.e. cases or boxes), please refer to our Relief Support & Resources page for resources and support assistance
  • Private businesses or nonprofit organizations who are not providing essential services or serving vulnerable populations in Denver in response to COVID-19


If you are a healthcare or medical service provider with a request for medical supplies, you may submit a request through our Medical Supplies for Organizations.

Looking to Donate?

Have a donation of Personal Protective Equipment (PPE), cleaning supplies, hygiene supplies, or other related items?

You can drop off these types of donations at:

  • The City is accepting donations of locally made face masks/coverings and hand sanitizer from citizens and commercial businesses in an effort to aid with current and anticipated supply shortages of these items.
  • PPE includes surgical masks, N-95 masks, hospital gowns, gloves, goggles, homemade masks, etc.
  • Cleaning and hygiene supplies include hand sanitizer, disinfecting solutions/wipes, toilet paper, soap, teeth cleaning supplies, and etc.
