Non Emergency Medical Vehicle Operator

For operators who engage in the transporting of sick and disabled persons in a non emergency medical vehicle

Owner must be at least 18 years of age. 

City employees are exempt from licensing.
 

How to Get a Business License

  • Individual Questionnaire  
  • Valid and current class A driver's license issued by the state
  • Affidavit of Lawful Presence
  • Valid State or Federally Issued ID 
  • Valid statement from a licensed physician reciting that the applicant has been examined by the physician within sixty (60) days preceding the date of the application and has been found to be free of any physical or mental defects that would affect the ability of the applicant to perform satisfactorily the functions of a non emergency medical vehicle operator (a current DOT card will suffice)
  • Proof that the applicant has completed a cardio-pulmonary resuscitation course and a standard first aid course approved by the Department of Public Health and Environment
  • Colorado Bureau of Investigations (CBI) Name Check (No older than 7 days)

 

Application Fee:  $25.00 

License Fee:  $25.00

CBI Name Check:  $6.85

 

Fees can be paid with cash, check,  money order or credit card.  Checks should be made payable to Manager of Finance. 

Once the background check has been accepted and approved, return to the Department of Excise and Licenses office.  Your license will be issued when the following actions are taken: 

  1. All documents required for the license are complete and submitted to the office.
  2. The background check is accepted and approved.
  3. Fees are paid in full.

Each licensed non emergency medical vehicle operator shall at all times when on duty carry the operator's license, identification card, and driver's license and shall, upon demand by an authorized inspector or a police officer, patient, or passenger, present all or any of the same for inspection.

Renew a Business License

Renew in Person
Come to the Department of Excise and Licenses office.

Request a renewal for your business license.

You will be issued a payment voucher.

Take the payment voucher to the cashier and pay the renewal license fee.

Return to the Department of Excise and Licenses office with your receipt for payment.

Your business license will be printed.

 

Renew by Mail
You can mail in your license renewal along with a check made payable to the Manager of Finance for the total city fees due. Be sure to include your Business File Number (BFN) on your check.

License Fee:  $25.00

Licenses must be renewed on or at least 60 days before the expiration date to avoid penalty fees. 

 

90 Day Grace Period

There is a 90 day grace period from the expiration date during which licenses can be renewed by paying the license fee and a penalty fee.

 

First 30 days of grace period:  20% penalty fee

Days 31 - 90:  50% penalty fee

After 90 days:  Licensees will be required to apply for a new license
