Financial Disclosure Reporting


Pursuant to DRMC 2-71(4)  and City Charter Section 9.1.1.E.(iii), (iv), (viii) (xiii), (xiv) and (xv)the following are City Officers:

The mayor, auditor, members of city council, manager of public works, manager of parks and recreation, manager of finance, manager of environmental health, manager of general services, manager of safety, city attorney, manager of human services, clerk and recorder, manager of aviation, manager of community planning and development, mayor's chief of staff, and any person appointed to a position and listed in section 9.1.1.E.(iii), (iv), (viii), (xiii), (xiv) and (xv) of the City Charter. Certified public accountants employed by the auditor shall not be considered officers.

NEW REPORT: Officer Disclosure of City Items (Semi-Annual)

All city officers and elected officials, on a semi-annual basis, are required to report items received from City agencies. They must report each item or thing received from the City with an estimated value of more than $50.

Due Date January 31 and July 31
Reporting Period

January 1 - June 30 is due by July 31.

July 1 - December 31 of the previous year is due January 31.

Officer Financial Disclosure Statement (Annual)

All city officers and elected officials are required to submit an annual financial disclosure of relationship to any business or trust in which a person is involved as a director, officer, owner in whole or in part, limited or general partner, employee, stockholder, trustee or beneficiary of a trust.

Officer Gift Disclosure Statement (Semi-Annual)

All city officers and elected officials, on a semi-annual basis, are required to report gifts and the solicitation of charitable donations of any value received from persons with whom the city has an existing, ongoing, or pending contract, business or regulatory relationship and over whom the officer may take direct official action as defined in section 2-52(b).

Reporting Period

January 1 - June 30 is due by July 31.

July 1 - December 31 of the previous year is due January 31.

Web Form The semi-annual gift report form is now online. The form will accommodate 60 entries and must be completed at one time.

 

City employee are required to report gifts and the solicitation of charitable donations valued at more than $50 and received from persons with whom the city has an existing, ongoing or pending contract, business, or regulatory relationship and over whom the employee may take direct official action as defined in section 2-52(b). This is an annual report. No report is necessary if the employee has not received gifts meeting the above criteria.

Submit To Your agency contact. Each agency designates a person to collect and store the employee disclosure statements. Please do not send your information to the City Clerk.
Web Form The annual employee gift report is now online. The form must be completed at one time.


Under Executive Order 134, all agencies under the Mayor are required to submit an annual Agency Gift Report for gifts (i.e. money, property, service or a thing of value worth $2,500 or more) during the previous calendar year. The report is due August 1.

Please submit the report to the City Clerk via email or bring it to the Office of the Clerk and Recorder, 201 W. Colfax Ave., Department 101.
